Medical insurance premiums; Approximately $300-$500 per month for 15 months of medical insurance premium is the minimum you should anticipate paying for insurance if Surrogate's insurance excludes surrogacy or if you need to reimburse her for her insurance policy. Surrogate must provide proof of insurance, including carrier name, policy information and premium amount. A fee to the insurance agent may be required for assistance in locating a policy for the surrogate. Paid directly to insurance company.
Life Insurance Approx. $400 - depends on location and age of surrogate. $250,000 term policy for Surrogate in case of accidental death. Cost approx. $400 depending upon age and location of Surrogate. Paid directly to the insurance company.
Medical Expenses
Psychological evaluation & MMPI (Minnesota Multiphasic Personality Inventory) Minimum of $400 and up to $2000 depending upon your clinic's requirements. Typically, one session with therapist, Surrogate and her partner to determine Surrogate's suitability, one session with Intended Parents and therapist if required or requested by clinic, and MMPI personality profile test administered by psychologist. Paid directly to the Psychologist.
Psychological Counseling $480 - $1200 for approx. 12 months.
Monthly sessions with our contracted agency therapist - $40/session. If the Intended Parents or Surrogate Mother select an outside therapist, rate will be approximately $100/session. This is not always needed -- determined on a case by case basis. Paid directly to the Psychologist.
Medical Evaluation by your Reproductive Endocrinologist (RE) $1500 - $2000.
Testing of Surrogates suitability for pregnancy & STD testing of spouse or significant other - varies by clinic. Approx. $1500 paid directly to the clinic.
IVF cycle fee and medications Approx $15,000-20,000.
Varies by clinic. Additional costs will be incurred if using egg donor or sperm donor as well.
Attorney Fees
Intended Parent Legal Representation $1000 - $2000
Draft, negotiate, and review contract with the Intended Parents, ensure signed originals are received from the Surrogate's attorney, document legal clearance to the physician and Agency. Usual charge is $1600-2000. Paid directly to the IPs attorney.
Surrogate Legal Representation $500 - $1000
Review contract with Surrogate and negotiate changes on her behalf. Up to $1000 of Surrogate's attorney's fees will be paid for by the Intended Parents for negotiating and reviewing a Surrogate agreement. Most agency lawyers charge $750 - $1000, Paid directly to the Surrogate's attorney.
Parental Establishment and Court Pleadings $1500 - $3000
Draft, review, obtain signatures, and file legal pleading papers with the Court. Arrangements for court appearances or default judgments will be made when required. Usual charge, $2000 - $2750. If the Surrogate must appear in court it will be closer to $2750. Paid directly to the IPs attorney.
Surrogate/Pleadings $500 - $1000
Review the legal pleadings in connection with the establishment of the Intended Parent's parental rights with Surrogate, and explain the effect of relinquishing her parental rights. Files all documents with the court. Paid directly to the Surrogate's attorney.
Court Filing Fees $600-1000
The fee varies depending on current court fees, and whether or not the Surrogate is married and which county the papers are filed in. All filing fees paid directly to the IP's attorney.
Surrogate Benefits
Surrogate Base Compensation $18,000 for a first-time Surrogate. Repeat Surrogates receive $23,000+.
Paid to Surrogate in 10 equal monthy installments over the course of the pregnancy beginning upon confirmation of pregnancy (usually 14-day blood test or detection of heartbeat).
Monthly Allowance Average $200 - $400/month
Surrogate expense reimbursement. In lieu of itemized costs, this fee includes reimbursements for mileage under 75 miles round-trip, lost wages prior to pregnancy, telephone calls, faxes, postage or federal express charges, housekeeping, childcare, and miscellaneous expenses such as pregnancy test kits, non-prescription vitamins, etc. (This fee is not in lieu of transfer fee.) Begins at contract signing to one month after birth: average $200 - $400 per month. Paid to Surrogate through escrow account.
Maternity Clothing Allowance $500 singleton, $750 multiples
Paid after completion of first trimester (12th week) to Surrogate through escrow account.
Mock Cycle Fee $500
Some Reproductive Endocrinologists will require this. The Surrogate will take medications to simulate a cycle to test thickening of uterine lining. Paid upon completion of mock cycle.
Dropped cycle Fee $400
If cycle is cancelled after the start of medications, through no fault of the surrogate's.
Embryo Transfer Fee $500/attempt
Paid after each embryo transfer procedure to compensate Surrogate for discomfort associated with the procedure. This covers lost wages and childcare for the embryo transfer trip (unless per the clinic, the trip is longer than 5 working days). Paid to Surrogate after transfer through escrow account. Does not cover per diem, airport parking, airport shuttles, any air travel/mileage/hotel expenses.
Invasive Procedures $500/procedure
Surrogate shall receive $500 per procedure for amniocentesis or other invasive prenatal diagnostic testing, early termination procedures, fetal reduction, or ectopic pregnancy to compensate for the discomfort associated with the above procedures. Fee does not include lost wages, childcare if needed, spouse's lost wages (if required), or air travel & travel expenses (per diem, airport parking, etc). Surrogate is allowed to have spouse or companion accompany her for any of these procedures and all necessary travel expenses/per diems related to the spouse/companion will be paid by IPs. Paid to Surrogate through escrow account.
C-Section $2,500
Surrogate shall receive an additional $2,500 (which can vary) at the time of delivery if she is required to deliver via cesarean section, to help compensate for the additional pain and suffering, added risks, and longer recovery time. Paid to Surrogate through escrow account.
Loss of reproductive capabilities; Loss of tubes $2000; loss of uterus $2000; loss of ovaries and/or complete hysterectomy $5000.
Compensation for the loss or mutilation of SM's reproductive functions (if not covered by Life insurance).
Multiples $2000/per child
In the case of multiples, the Surrogate receives additional payments of $2000 for each additional baby delivered. Paid to the Surrogate through escrow account. These payments are typically broken up monthly starting at the 4th month of pregnancy.
Lost Wages Approx. $3,000 for working surrogates.
Lost wages are determined by actual after-tax earnings - can be much less depending upon circumstances, disability insurance and actual earnings. Estimated lost wages for Surrogate (approximately 6 - 8 weeks with normal pregnancy and post partum recovery) and spouse (three to five days to help assist during transfer, pregnancy and/or delivery). If Surrogate is a stay-at-home mom or unemployed, no lost wages are paid to Surrogate (lost wages for spouse still apply). Surrogate and husband must provide proof of actual lost wages. Additional lost wages may also include time away from work for doctor appointments and doctor ordered bed rest. Paid to the Surrogate through escrow account.
Lost Wages for Spouse
There may be lost wages for spouse (after tax earnings as verified by recent pay stub) for embryo transfer trip, invasive procedures, emergencies and if necessary for birth (approx. 5 - 7 days)
Travel Expenses and Per Diem $50/day/person
If travel is necessary for medical evaluation and transfer - $50/per day /per person. Spouse or companion often accompanies surrogate on both trips. IPs are also responsible for hotel, parking, airfare, lost wages for GS and companion, travel to and from airport etc, if necessary.
Air Travel
Round-trip airfare to state for necessary clinic appointments and the transfer: economy fare. Local travel and accommodations to be supplied by/paid for by IPs. For transfer and any invasive medical procedures (i.e. CVS, amnio, selective reduction, D&E), roundtrip airfare for spouse and or companion if requested.
Local Travel $.43/mile
GS's traveling more than 75 miles roundtrip by car to be reimbursed at a rate of .43 cents per mile. For doctor, clinic visits. To be paid from escrow account.
Maid Service and Child Care
If put on doctor ordered bed rest (during pregnancy, miscarriage, termination/selective reduction procedure, etc.) Maid service - up to $100 for each week on bed rest and in the event of needing child care, max. of $100/day. (Receipts needed for reimbursement) This would be separate from monthly allowance.
Breast Milk Pumping $0 - $250/week
To be agreed upon by GS and IPs. Surrogate can determine length of time and amount she will receive. Standard fees are between $0 - $250/week plus IPs pay cost of pump rental and all shipping charges.
Miscellaneous
Escrow Account Set-Up Fee Approx $700 - $1000
Includes Bank Fee to set up account and Check Fee - $10/check. Maintain account, disperse Surrogate compensation and expenses on approval. Paid to escrow account company.
Criminal Background Check $125
Paid to agency by the Intended Parents upon signing of agency contract.
TOTAL COSTS:
Many Intended Parents ask us to estimate the costs of a surrogacy journey. Unfortunately, each case and scenario is different. Complications arise in some cases and not in others. Some people need an egg donor while others don't or already have frozen embryos. As you can see, the situations can vary greatly. But based on our personal experience in this industry, we anticipate the range to be from $60,000 to $110,000 (which would be the worst case scenario with foreseeable maximum expenditures.) Again, there are no guarantees, but we always try to do our best to keep costs under control while making sure that the surrogate receives the compensation and reimbursement she deserves. All of the above costs are subject to change as the professionals that we work with may, at any time, increase or lower their fees, and individual Surrogates may have their own fees which reflect their unique circumstances, and the IP or GS's lawyer may make additional contract recommendations.
A NOTE ON COSTS:
Whether you choose to work with an Agency program or with a surrogate independently, you will incur most of the above-mentioned fees such as a benefit package, which is in addition to her base fee. Even though money can be a part of the incentive, we try to work with women who are not Surrogates solely for financial gain, but are involved because their hearts tell them it is the right thing to do, to give the gift of life to a couple who cant do so on their own.
Agency Fees
As former Surrogate Mother, I know how overwhelming and expensive it can be for the Intended Parents pursuing parenthood through surrogacy. In an effort to keep this journey affordable and thereby available to as many Intended Parents as possible, we do not mark up any of the services or referrals that we make including the psychologist for Surrogate evaluation, lawyers for both IPs and GS, escrow accounts, etc. Unlike many of the agencies, we do not add on a consultation fee of $250 - $1000 for our time. We feel that you should be able to make an educated decision about your agency choices without having to start writing checks. The journey is expensive enough and we really do want to help you get off to the right start, whether it is with us, another agency, or on your independent surrogacy journey.
We do our best to keep things affordable, but that is often out of our control. What we can control is the amount of personalized service we can give you, and we know, based on the other agencies, including the most high-profile and expensive ones, that our service to both our IPs and GSs far exceeds other agencies. Our business is our passion.
The following payment schedule is effective as of August 1, 2008.
Four Thousand Five Hundred Dollars* ($4,500.00) payable in three (3) payments:
One Thousand Five Hundred Dollars ($1,500.00) due upon signing our agency agreement
One Thousand Five Hundred Dollars ($1,500.00) due the day after you and your Gestational Surrogate sign your Surrogacy Contract
One Thousand Five Hundred Dollars ($1,500.00) due the day before the Gestational Surrogate's First Embryo Transfer
